Kentucky State Police Investigate Perry County Fatal Collision

HAZARD, Ky. (September 11, 2025) – On Monday, September 8, 2025, at approximately 2:44 P.M., Kentucky State Police (KSP) Post 13 in Hazard responded to a report of a two-vehicle collision with injuries on KY-15 in the Hazard community of Perry County.

Troopers, along with Emergency Medical Services (EMS), arrived on scene to find a Ram pickup truck and a Honda Civic involved in a head-on collision. Preliminary investigation indicates that Roger D. Combs, 78, of Hazard, was operating a black Ram pickup traveling northbound on KY-15 when the vehicle crossed the median into the southbound lane, striking a black Honda Civic driven by Edward Goins, 50, of Sassafras.

Mr. Combs was transported by EMS to Hazard ARH Medical Center. Mr. Goins was airlifted to Johnson City Medical Center, where he remained in critical condition.

On Wednesday, September 10, 2025, KSP Post 13 was notified by medical staff at Johnson City Medical Center that Mr. Goins had succumbed to injuries sustained in the crash.

Toxicology reports are pending. Both drivers were wearing seat belts at the time of the collision. The investigation remains ongoing and is being led by Detective Scott Caudill, who is also conducting a collision reconstruction.
